“This safety questionnaire has been designed by two Behavior Analysts for the explicit purpose of evaluating potential risks and safety concerns associated with caring for children or adults with disabilities. The questions included are intended to assist providers working with vulnerable populations and/or parents/caregivers in identifying and addressing potential hazards and safety skills deficits” (Foxman & Kelly, 2023).
